dc.description.abstract Abstract Introduction White spot lesion (WSL) adjacent to orthodontic appliances is a common side effect of the orthodontic treatment. Aim This systematic review aims to summarize randomized clinical control trials (RCTs) that evaluate the effectiveness of fluoride varnish application in prevention and treatment of WSL in comparison to placebo or different forms of remineralizing agents in orthodontic patients. Material and Methods Systematic search for RCTs on humans published in English language was performed in PubMed. PICOS was: P; Orthodontic patients during or after treatment. I; Fluoride varnish C; Placebo or any other remineralization agents. O; Presence and/or absence of WSL. S; Randomized clinical trials. Results After screening of titles and abstracts of 152 articles, 6 studies were identified as eligible for this systematic review. Three of the included studies showed significant intergroup differences and one of the studies was significant only using DIAGNOdent as a measure, but not significant using visual assessment. The two remaining studies had non-significant outcomes regarding fluoride varnish application for prevention and/or treatment of WSL in orthodontic patients. The quality assessment revealed a low risk of bias for the majority of the elements in the included studies. Conclusion This systematic review has shown the effectiveness of fluoride varnish in preventing and/or treating of WSL in orthodontic patients in some studies, but the limited clinical trial evidence make it difficult to draw a definitive conclusion. Future high-quality RCTs with rigid eligible criteria and homogenous procedures are necessary in order to achieve more and solid clinically significant evidence-based guidelines. en_US
